Service coordination helps organize care, connect services, manage communication, and advocate for the participant. It ensures the plan stays on track and families do not have to manage everything alone.

Do you work with hospitals, discharge planners, and social workers?

Yes. We collaborate with healthcare professionals, rehab facilities, and social workers to support safe transitions, strong care planning, and community-based support.
